  • Darling Downs and West Moreton PHN is seeking a range of diverse community perspectives to ensure our investment in health is focussed on where it is most needed.

    Expressions of interest are now open to be part of the community advice panel providing input into healthcare planning for the Darling Downs and West Moreton regions.

    Members are sought for:

    • Darling Downs Community Advisory Committee
    • West Moreton Community Advisory Committee
    • Community Advisory Database to provide input on topics of interest.
  • About the Darling Downs and West Moreton PHN

    Darling Downs and West Moreton PHN is a federally funded organisation who works together with the primary health sector to improve health outcomes for all Australians. 

    PHNs have been established with the key objectives to:

    • increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
    • improve coordination of care to ensure patients receive the right care in the right place at the right time.

    The PHN includes metropolitan, urban and rural regions, with a combined population of over 530,000 people.

  • Community Advisory Committees

    Through this EOI, Darling Downs and West Moreton PHN are calling for Expressions of Interest from local stakeholders across the Darling Downs West Moreton region who are well-placed to actively participate in providing community input health care priorities.

    The purpose of the Community Advisory Committees is to bring together diverse perspectives across   a range of population and disease groups, particularly focused on those populations who are over‐ represented in population health data and contribute to significant avoidable use of   hospitals.

    The purpose of the Community Advisory Committees is to represent the communities of the region and assist the DDWMPHN with strategic input, planning and communication as well as participating in the development of local clinical pathways.

    It is important to note that members are representative of their community, and have been selected based on their personal and related experiences of the health system.

    Due to the differences across communities, there are two Community Advisory   Committees:

    • Darling Downs
    • West Moreton

    The Committees meet bi-monthly and members are remunerated for their participation in line with the PHN Remuneration Policy. For further information on the Committee, please request the Terms of Reference by contacting communications@ddwmphn.com.au.

  • Community Advisory Database

    The PHN also regularly seeks input on specific health topics, sometimes in particular regions. This includes four Talk About topics each year that canvass views on healthcare in our region. This is not a paid position.

  • Selection criteria

    To express your interest in representing your community’s views on healthcare across the region, please complete the below form.

  • Strong embeddedness within the Darling Downs or West Moreton region, and high awareness of local health needs (500 words)

    Applicants are required to demonstrate their:

    • strong understanding and/or experience of the regional health needs, including within specific community cohorts
    • awareness of the demographic profile of the region, which is applied to enhance the applicant’s knowledge of needs, gaps, and opportunities of the health landscape; and
    • strong partnerships and engagement within your community including with people who have accessed services and their families and carers, or people with a lived/living experience of healthcare.
  • What do you see are the benefits of community input into healthcare priorities? (500 words)

    Applicants are requested to provide an outline of their understanding community-centred care including:

    • examples of previous involvement in community advisory groups
    • insights and expertise that the applicant was able to contribute to these groups, and
    • identification of qualities and approaches that the applicant can bring that are crucial contributions to a Community Advisory Committee.

    The PHN is very keen to have a diverse range of people involved in our advisory committees. To enable us to appropriately support you to be an active member of the Committee, please detail below any requirements for you to participate in the Community Advisory Committee including:

    • preferred times for meetings to enable attendance
    • any accessibility requirements including disability access, language or technology support required
    • work or other commitments that may impact your capacity to attend meetings on a regular basis
    • transport requirements
